In order to obtain a copy of your medical records, please complete a Release of Protected Health Information form.

Once completed, please mail, fax, or drop the form off to your nearest health center.

Screening means checking your body for cancer before you have symptoms. Getting screening tests regularly may find breast, cervical, and colorectal (colon) cancers early, when treatment is likely to work best. Lung cancer screening is recommended for some people who are at high risk.

The Teen Awareness Group helps teens make informed and responsible choices about sex, relationships and contraception. The program also offers free condoms and sexual health resources at all Hudson Headwaters health centers; simply ask for a “TAG Bag” at the front desk.
