Care managers work alongside the primary care, specialty care, and behavioral health care team to provide health coaching and coordination. They help patients better understand their conditions, health care goals, and medications. Homeward Bound

If you (or a family member) are over 65 or disabled, with chronic health problems that keep you homebound, the Homeward Bound team may be able to help. Homeward Bound is a home-based primary care program for Hudson Headwaters’ frail and homebound patients living in the greater Glens Falls area. Our goal is to bring primary health care to you at home when your illness and/or health condition make it too challenging for you to travel to your primary care doctor at your Health Center.
