Pathways Homeward Bound: Bringing Health Care Home

We offer home-based primary care for patients with chronic and serious illnesses who have a difficult time leaving the house. Our team is here to help you feel better and to improve your quality of life.

Homeward Bound is for homebound patients living in the Greater Glens Falls, Warrensburg, Hadley, Corinth and Lake Luzerne regions. To access Homeward Bound, you must be a Hudson Headwaters primary care patient with a referral from your primary care provider.

What can I or my loved ones expect from the program?

Our team will:

• Work with your other medical providers.
• Connect you to skilled home health and/or hospice services when needed.
• Make urgent house calls to avoid emergency room visits or hospital stays.
• Help you decide what kind of care will meet your goals for your own health and wellbeing.
• Grow a trusting connection with you and your family and caregivers
