Log in to the Hudson Headwaters Patient Portal to view your test results online. If you are not yet registered for the Patient Portal, please create your account before your test. Please note: If you choose not to use the Patient Portal, notification of your test results may be delayed.
It is important to get tested if you feel ill and/or have had close contact with someone who is either COVID-19 positive or suspected positive. You should also get tested if you have recently attended a large gathering, or have recently traveled to a high-risk region, even if you are not experiencing symptoms.
Masks are currently optional for staff, patients and visitors of Hudson Headwaters Health Network facilities. We ask that you wear a mask if you have respiratory symptoms, fever or a rash. Masks are available free of charge at all health centers.
After your appointment has been scheduled, you’ll receive email and text notifications 24 hours prior to your appointment and again 2 hours prior. These notifications will include a link you can click to connect to the telehealth appointment system, Qure4u. Make sure you have a computer, tablet or smartphone with the camera on before joining the appointment.

Telehealth visits are billed to your insurance, and patients are responsible for the charges incurred (copay, deductible, etc.). Please call your insurance company with questions about your specific insurance plan and coverage to determine if you will incur any out-of-pocket costs for a telehealth visit.
On the child’s 18th birthday, the access level for all family members with full access to the patient record changes to billing-only access.
On a desktop computer: In the upper right corner of the Patient Portal, next to your name, click the down arrow and select from the patient list.
On a mobile device: Navigate to the “viewing” field at the very bottom of the screen. Click on the dropdown menu to select the patient.
On a mobile device in “full site” view: In the upper right corner of the Patient Portal, next to your name, click the down arrow and select from the patient list.
To use the same email address for the Patient Portal, patients must choose one person to be the Patient Portal account owner. Additional patients will be added as family members. The Patient Portal account owner has access to all family members’ patient records when logged into the Patient Portal. For each patient to be the owner of a Patient Portal account, each patient must establish his or her own account using a unique email address.
Use the “Messages” section mid-way down on the Patient Portal homepage, not the “Messages” option located in the upper right corner of the homepage. After you create your message, click on the “Attach Files” link to select your document from your computer or phone.
Click on “My Health” and then “Health Reminders.”
On the left-hand side of the Patient Portal homepage, navigate to the bottom of the list and select “Test Results.”
To view the full desktop version of the Patient Portal, tap “Full Site” at the bottom of any page within the Patient Portal.
All communications between you and your provider’s office are carried over a secure, encrypted connection. This secure connection utilizes industry-standard Secure Socket Layer (SSL) encryption to ensure data transmission as well as server-side digital certification authentication. To prohibit unauthorized access, all medical information is stored behind our firewall in our electronic medical record system.
You should always make sure that the email address on file for your account is accurate, as notifications from the portal are sent to the email address on file. Make sure to sign out of your account each time you are finished using the portal.
Any active patient is eligible to register for and use the Patient Portal. If you are authorized, a family access account can be created that will allow you to access select family members’ health information.
Hudson Headwaters patients have secure online access to most of their personal medical information through our Patient Portal (click here to sign up). You can view your medication list, allergies, immunizations, appointment history and most test results. You may also complete and update medical forms, exchange messages with your primary care provider team and request non-urgent appointments and medication refills. Caregivers may also be given access to medical information.

Patients are responsible for co-payments, unmet deductibles and non-covered services at the time of their visit. Payment in full is also expected at the time of your visit if Hudson Headwaters does not participate with your insurance plan.

Yes, financial assistance is available. We offer a Sliding Fee Program that discounts the cost of medical and dental care at our health centers (including primary and specialty care, many dental services, prescriptions and X-rays). You may be eligible for the program based on your household/family size and how much you earn. We welcome you to complete the required forms in your New Patient Registration packet and bring them with you. Or you may complete the forms when you arrive at the health center for your visit. Please arrive at least 15 minutes early and bring your insurance card, a photo ID and a current medication list.
We recommend choosing the health center nearest to your home or work—whichever is most convenient. Click here to see our locations. When you call, we will ask if you want to make an appointment with a specific provider. Visit our providers page to learn more about our clinicians, including who is currently accepting new patients. If you do not have a specific preference, we will set you up with an available provider who best meets your health care needs.

If you have an urgent medical concern, we invite you to “walk in,” without an appointment, at our urgent care centers: Health Center on Broad Street in Glens Falls or Warrensburg Health Center.

Appointments must be canceled (or rescheduled) by phone at least 24 hours in advance. You may be subject to a $25 “no-show” fee if you do not call to cancel or reschedule your appointment.
Please refer to this page for health center hours; staff are available to schedule same-day appointments when the center opens. When the center is not open, you can request an appointment via a message through the Patient Portal. Nursing staff will then contact you regarding appointment availability.
All Hudson Headwaters health centers have same-day appointments available based on patient need, but most do not routinely offer urgent care. If there are no more same-day appointments available at the time of your call, you may be referred to another nearby health center or to one of our two urgent care offices: Health Center on Broad Street in Glens Falls or Warrensburg Health Center. Each urgent care location is open seven days a week, with evening hours Monday through Friday in Glens Falls and Monday through Saturday in Warrensburg.
Yes. Interpreting services are utilized at all Hudson Headwaters health centers. This free service instantly connects patients to a live, medically trained interpreter via a computer during their office visit to ensure a better patient experience. Please notify our patient services staff if interpretation services are needed during your visit.
Hudson Headwaters provides medical services to many nursing home/rehab facilities in the Adirondack region of Upstate New York.
Most referrals will be handled during your visit. If you request a referral for additional or specialty services (not at the time of your visit), we will ask for the name of the specialist, the date and reason for your upcoming visit and appropriate insurance information.
Hudson Headwaters delivers test results in several ways: through the Patient Portal, mail and our “Results Call” system – an automated phone or email message.
Please request non-urgent, non-controlled medication refills by calling your pharmacy. This is the fastest and most efficient method of requesting refills. If you need provider approval for a renewed prescription, you can still call your pharmacy, send a message online via your Patient Portal or call your health center. We process prescription refill requests Monday through Friday and within three business days.
Our telephone lines are open 24 hours a day, seven days a week. If your local health center is closed, your call will be automatically forwarded to one of our urgent care facilities. After they close, calls are forwarded to an on-call provider.
If your condition requires emergency care (chest pain, severe difficulty breathing, trauma, loss of consciousness, etc.), please call 911 or seek treatment at the nearest hospital emergency room. If you are unsure if your condition requires emergency care, call your local health center.
For non-life-threatening conditions, Hudson Headwaters’ urgent care services can save you a trip to the emergency room. Our urgent care centers in Glens Falls and Warrensburg let you “walk-in” without an appointment. At our other health centers (which don’t routinely offer urgent care), walk-in visits are on a first-come, first-served basis (after scheduled patients). Calling ahead is recommended for those health centers that do not routinely provide urgent care.
