- Chester-Horicon Health Center
- Family Health at Lake Placid
- Indian Lake Health Center
- Moriah Health Center
- North Creek Health Center
- Schroon Lake Health Center
- Ticonderoga Health Center
- Warrensburg Health Center (primary care and specialty care)
Warrensburg Health Center Urgent Care will remain open with normal hours of operation.
