The NYS Board of Pharmacy has approved the opening of Hudson Headwaters Pharmacy at Salem, located at 1830 County Route 64 inside Salem Family Health.

As required by New York State, the pharmacy must open to begin securing participation with commercial insurance, Medicare and Medicaid. During the soft open phase, the Pharmacy at Salem is self-pay. Medicaid approval is pending and may take up to six months; participation with Medicare and commercial payers should be sooner. Individuals may call the pharmacy for a current list of accepted insurance plans.

“As a community health center, providing comprehensive access to health care services is at the heart of our mission. This Pharmacy at Salem will add convenience for Washington County families and support overall community health,” said Tucker Slingerland, M.D., CEO of Hudson Headwaters. “We are pleased to announce the soft launch and look forward to updating the greater Salem community as we work through the required process to secure private and public insurance participation.”

Patients and community members are invited to call 518-320-7619 or stop by the Pharmacy at Salem to learn more and explore options for filling their prescriptions during the soft open phase. Pharmacy hours are Monday – Friday from 8 a.m. to 5 p.m. The pharmacy is closed for lunch from 12:30 p.m. – 1 p.m. Automated phone and website systems are available 24 hours a day, 7 days a week. The Pharmacy at Salem is part of the 14,300-square-foot Salem Family Health primary care facility that opened in May 2025. The center includes on-site lab services operated by Glens Falls Hospital and the Stewart’s Shops/Dake Family Teaching Kitchen, a state-of-the-art space offering free community nutrition classes thanks to support from the Mary McClellan Foundation.

The pharmacy in Salem is the Network’s second; Hudson Headwaters Pharmacy at Moreau opened in 2024.
