    New Champlain Family Health To Open July 10, 2017

    New Home, New Name, Same Great Care

    Formerly known as North Country Family Health located at 481 State Route 11, Champlain, NY the newly re-named Champlain Family Health center will be officially opening its doors just down the road, to patients on July 10th at 828 State Route 11, Champlain NY. The health center is open six days a week, hours of operation will be Monday through Thursday from 8am-8pm, Friday 8am-6pm, and Saturday 8am-4pm. Patients have been mailed a letter informing them of the new location, name change, and brochure detailing services and welcoming them to their new health center.

    “We have also added the location and name change to our reminder calls,” said Deborah Bardin, VP of Health Center Operations. “We’re excited for our providers, staff, and most importantly, our patients to enjoy this beautiful new state-of-the-art building.”

    The new health center will have 24 primary care exam rooms, three behavioral health counseling rooms, a procedure room, and additional space for expanded care management, teaching, and specialty care services.

    The new Champlain Family Health center offers: Family Practice, Internal Medicine, Pediatric and Adolescent Medicine, Preventive Medicine, Physicals, Immunizations, Cancer Screenings, Women’s Health Services, Same Day Appointments, Minor Surgery, Phlebotomy (Blood Draw), Care Management and Social Work, Sports Medicine, Osteopathic Manipulation, and Behavioral Health Services.

    “We are grateful for Hudson Headwaters, Dr. Racine, and his team of providers and staff who have worked so hard for so long to provide quality care to our region.” said Celine Paquette, Board Member. “It’s wonderful to see such a beautiful finished product for our community.”

    Champlain Family Health provides care to people living in Champlain, Rouses Point, Altona, Chazy, Moores, and West Chazy, a 500 square mile rural area with more than 17,000 residents.

    “Our goal has always been to expand access to health care to better serve the area,” Dr. Slingerland, CEO said. “We are excited to play a role in expanding the opportunities patients have in gaining access to quality care.” To that end, Hudson Headwaters has reached an agreement with the University of Vermont Health Network (UVMHN), which includes CVPH, to bring laboratory and radiology services to the new health center.
