
Partnering With Patients to Improve Care
At Hudson Headwaters, we believe strong relationships with our patients lead to better care. That’s why we created the Patient Advisory Council (PAC), a group of patients who meet with our team to share ideas, offer feedback and help improve the care we provide.
What is the Patient Advisory Council?
The PAC is made up of patients who meet with Hudson Headwaters staff to share their experiences. Based on real-life patient perspectives, members help us understand what’s working well and where we can improve.
What Does the Council Talk About?
Council members suggest practical ways to improve how we deliver care throughout the Network. Topics may include both non-clinical and clinical areas of care:
- Scheduling and check-in
- Online tools and patient portals
- Health center comfort and accessibility
- Care from nurses and providers
- Available services
- Overall visit experience
Member Expectations
All PAC members are expected to:
- Attend two 60-minute meetings per year, held virtually (via Microsoft Teams) or in person at a Hudson Headwaters location.
- Take part in follow-up discussions or small projects between meetings.
- Commit to serve on the Council for at least one year.
