We are pleased to welcome you at any of our health centers in northeastern and upstate New York. We hope the following questions and answers will help you as you join us. Thank you for your interest in Hudson Headwaters.

For routine medical concerns, please call one of our health centers during normal business hours to make an appointment.

If you have an urgent medical concern, we invite you to “walk in,” without an appointment, at our Health Center on Broad Street in Glens Falls, or Warrensburg Health Center on Main Street.

We recommend choosing the health center nearest to your home or work – whichever is most convenient for you.  Click here to see our locations.  When you call, we will ask if you want to make an appointment with a specific provider.  (Click here to learn about members of our medical staff.)    If you do not have a specific preference, we will set you up with an available provider that best meets your health care needs.

We welcome you to complete the required forms in your New Patient Registration packet, and bring them with you.  Or you may complete the forms when you arrive at the health center for your visit.  Please come at least 15 minutes early to any health center visit and please bring your insurance card, a photo ID, and a current medication list.

We participate with most major insurers, including Medicare and Medicaid. A list of participating health plans/insurers can be found here.

Patients are responsible for co-payments, unmet deductibles, and non-covered services at the time of their visit.  Payment in full is also expected at the time of your visit if Hudson Headwaters does not participate with your insurance plan.

If you don’t have insurance, you may be eligible for financial assistance.  We will help you enroll in appropriate programs.
