You can now receive the same great care from Hudson Headwaters from the comfort of your home, or wherever you are! Our new telehealth services offer current and new patients an easy way to visit with their providers using a personal computer, smartphone or tablet. Patients can choose either a phone visit or a visit using video and audio. Many of our patients find they prefer video and audio as this allows for the same kind of face-to-face conversation that would normally happen in person.

Telehealth Appointment Types - available for existing and new patients!

Telehealth appointments are available for many primary care, specialty care and follow-up appointments. However, telehealth may not be appropriate for every type of appointment (worker’s comp and annual wellness visits are excluded). Please reach out to your health center with your health-related concerns as you normally would. Staff will provide guidance regarding which appointment types to defer for now, which appointments to schedule via telehealth and which may require an in-person visit. If you are experiencing a medical emergency, call 911.

By scheduling your telehealth visit you:

  • Agree to participate in a telemedicine visit with a Hudson Headwaters provider. The telemedicine session will not be audio or video recorded at any time.
  • Understand that this service is not the same as a direct patient/healthcare provider visit, because you will not be in the same room as the healthcare provider performing the service.
  • Agree to permit your healthcare information to be shared with other individuals for the purpose of scheduling and billing.

Telehealth visits are billed to your insurance and patients are responsible for the charges incurred (copay, deductible, etc.). Please call your insurance company with specific questions on your specific insurance plan and coverage to determine if you will incur any out-of-pocket costs for a telehealth visit.

How It Works

We are utilizing a telehealth platform called MyCarePlan to conduct secure telehealth appointments. Using MyCarePlan requires you to have access to either a computer, smartphone or tablet (e.g. iPad), internet access and an email address. We recommend connecting to WiFi to avoid incurring mobile data charges.

If you do not have access to the equipment needed for a telehealth appointment, our providers are also able to perform some appointments via telephone. Please call your health center to schedule a telephone-only visit.

Before your first scheduled telehealth visit, you will receive instructions via email. The email will provide you with links for downloading the HHHN app or to the browser version.  Please see the link above and to the right “Telehealth setup instruction” for details on how to navigate the email and activate your account. Please also review the “How to do a Telehealth Call” link for further instructions on how to test the camera and sound on your device.

When it’s time for your appointment open the HHHN app (or log in from your computer) then tap or click on “Start Video Call”. Wait until your provider starts – don’t navigate off this screen. You’ll be able to see and talk with your provider (and nurse). When your visit is over, close the app or browser page.
