Primary or general health care is provided by physicians who specialize in family practice, pediatrics, or internal medicine. Hudson Headwaters offers:

Specialty Care is most widely available at our Warrensburg Health Center, which serves as the referral hub for the Network. How to Get Care

For routine medical concerns, please call one of our health centers during normal business hours to make an appointment. We encourage you to ask for the provider of your choice. If you must cancel an appointment, please call as early as possible so that we may offer that time to another patient.

If you have an acute illness, you can often be seen the day you call. Please let the receptionist know that you are seeking a same day appointment. In some instances, you may be referred to our Urgent Care staff at the Health Center on Broad Street, the Ticonderoga Health Center or the Warrensburg Health Center.

Patients whose health condition may require emergency care (chest pain, severe difficulty breathing, trauma, loss of consciousness, etc.) should seek treatment at the nearest Emergency Room. Call 911. If you are unsure if your condition requires emergency care, call your local health center.

Patients who come for care without an appointment are handled on a first-come, first-served basis (after scheduled patients), with the exception of medical emergencies.

24-Hour Medical Call: Our telephone lines are open 24 hours a day, seven days a week. You can always reach a health care provider by calling your local health center. If your local center is closed, your call will be automatically connected to the provider on call.

Hospitalization: Hudson Headwaters physicians care for our own patients in the hospital setting. A Hudson Headwaters physician is on call at the Glens Falls Hospital at all times. Should you need hospital care, our doctor will consult with any specialists needed to ensure that you receive the best possible care.

Referrals/Additional Services: If you call requesting a referral for additional or specialty services (not at the time of your visit), we will ask for the name of the specialist, the date and reason for your upcoming visit, and appropriate insurance information.

Test Results: Hudson Headwaters will notify you of significant test results via mail or at the time of your next appointment. In special cases, you may be contacted by telephone.

Prescription Refills: For assistance refilling current prescriptions, please call the health center where your prescription was written. We will process your request within 48 hours. (PLEASE NOTE:  QUEENSBURY, MOREAU and CHESTER HEALTH CENTER PATIENTS:  This process has changed.  Please call your pharmacy directly for prescription refills of non-controlled medications).    Please visit "Pharmacy Services" on this website for more information on our pharmacy programs.

Payment Policies

Hudson Headwaters welcomes all patients regardless of their insurance status. We participate with most major insurers, including Medicare and Medicaid. Please bring your insurance card with you for every visit. We also offer a Sliding Fee Program for the uninsured (Call 1-888-624-0226 for more information).

You are responsible for paying for non-covered services, co-payments or unmet deductibles in full at the time of your visit. Payment in full is also expected at the time of your visit if Hudson Headwaters does not participate directly with your insurance plan.

Hudson Headwaters may add a $10 fee to your bill if non-covered services, co-payments or unmet deductibles are not paid in full at the time of your visit.

Hudson Headwaters also provides residents and businesses with care for Worker's Compensation, school and sports physicals, and routine, employment and wellness physicals.

Payment Assistance Programs

Hudson Headwaters wants you to get the health care you and your family need. We offer assistance through our Sliding Fee program that matches the cost of your care with your ability to pay based on your income and household size. Sliding Fee patients may also be eligible for discounts on prescription drugs.  For more information about our Sliding Fee program, please call 1-888-624-0226. 
 
Hudson Headwaters can also help you enroll in other programs for low-income patients that are listed below. To register or to find out more about the programs listed below, please call our toll-free information line at 866-708-2912.

Child Health Plus
A low or no cost health insurance program that covers most medical expenses for children under age nineteen (19) or pregnant women who live in New York State. You may qualify for Child Health Plus A (Children's Medicaid program) or Child Health Plus B.

Family Health Plus
Available to adults between the ages of 19 and 64 who do not have health insurance, but have incomes too high to qualify for Medicaid.

Medicaid for Adults
A health insurance program that offers adults complete coverage for all their health care needs. No monthly payment is required.
